1. Access the GoZen Return Form

2. Fill in the Contact Details
In the Contact Details section, provide the following information:
- Company Name: Enter your company’s official name.
- Contact Person: Fill in the first and last name of the person submitting the return request.
- Email Address: Provide a valid email address where we can send updates regarding your return request.
- Phone Number: Include a valid phone number with the correct country code for further communication.
- Preferred Contact Method: Select your preferred method of communication (e.g., phone, email).
3. Provide Return Details
In the Return Details section, complete the following fields:
- Product Type: Select the type of product you are returning from the drop-down menu.
- Model Number: Provide the model number of the product.
- Serial Number: Enter the product’s serial number (this is required).
- Reason for Return: Select the appropriate reason for returning the product (e.g., damaged, not functioning, incorrect item, etc.).
- Associated Ticket Number (If Applicable): If your return is related to an open support ticket, enter the associated ticket number.
- Description of Issue (If Applicable): Provide a brief description of the problem, especially if the return is due to a defect or issue with the product.
- Condition: Indicate the current condition of the product (e.g., new, used, damaged).
- Original Packaging?: Specify whether the product is still in its original packaging (Yes/No).
- Desired Resolution: Select your preferred resolution (e.g., exchange, refund, repair).
- Attach Media: Upload any relevant images or documents that may assist in the processing of your return request (e.g., pictures of the damage, receipts, etc.).
4. Submit the Form
Once all fields are completed, click Submit at the bottom of the form. You will receive a confirmation email that contains a summary of your submission.

- Acknowledgement: After submitting the form, you will receive an acknowledgement email with the details of your return request.
